Already following a vet-directed food trial?

The free snapshot covers one week of observations. The 8-Week Dog Food Trial Kit provides a longer record and a shared household plan for the trial your vet recommends.

Your veterinarian chooses the diet, tests and treatment. These resources help you organize notes.

Can this tell me what is causing the itching?

No. The snapshot records what you observe. Itching can have different causes, and your veterinarian needs to assess your dog. Do not change food or medication based on a score.

Do I need to finish all seven days?

No. Contact your vet when you are concerned and bring whatever notes you have. The log is not a reason to postpone an appointment.

How do I use it on my phone?

Save the PDF to your device, then open it in a PDF reader that supports fillable forms. Type your notes, save again and reopen to check they are there. You can also print and write by hand. This page does not store your dog’s notes.

What if I miss a day or cannot choose a score?

Leave a missed day blank rather than guessing. Use the same personal scale each day: 0 means none; 10 means almost nonstop scratching, licking or chewing. Add a short description of what you saw. A score cannot identify the cause.
